An application, interview and certificate of approval are required for each purchase or lease in the community. The application fee is $250.00 ( cashiers check or money order only). One check in the amount of $150.00 payable to Forest lakes HOA and one check in the amount of $100.00 payable to Banyan Property Mangement, Inc. and applications may be downloaded, using the below button. An interview is required and may be scheduled using the "Service Request" button on the Forest Lakes homepage
